I did. I've been having a hard job converting the file from .rtf to .jpg, so I can't show it here, but it contained mostly details already here. The only other important things are the test question and answer (Q-Mother's maiden name? A-Janice Louise Anus) and the signature of the agent (Mickey Mouse).
