... it's so hard to figure out whether it means the date on the "winner form" or the date that you're filling out the form. We wouldn't want to fill out the form incorrectly. That just wouldn't do.

Fortunately, my good friend has now given me the correct instructions! I wonder if I still have a copy of the form?
