The UK government has recently introduced the GIT Act, which means that anyone making a general international transfer has to fill in these forms before said process can be completed.

Tell me what you think, even if you think they are rubbish! - They're my first attempt so don't expect too much...

For some reason the L's and some I's look like they are in bold, not quite sure why, but when you print them they come out fine...

thanks max - yeh i thought the terrorism questions may be going a little too far, but hey - would be interesting to see if anybody could convince their mugu to fill it in

As for the bold L it seems to be something to do with converting fonts to outlines. If you don't convert it then it should show up just fine. Like you say though, it prints fine.

But what if anyone answers the Should I stay or should I go? - question with I should go!? Will the transfer being seized by the relevant authority then? Or just restarting from all over again for not taking the act of form filling seriously enough? Smile

As has been mentioned, the bunny thing is a bit over the top, but what the hey....give it a shot and fine tune from there.

It looks almost exactly like the over-complicated, confusing forms the UK Gov really issue. I'll def try to use it on a lad and post the results.

Fantastic forms, MrMe. I can't wait to use them.

Couple nitpicks: Do you really want them to use "blue or blank ink"?

I love the bunny picture. I wonder if you replaced the word "bunny" in the text with "subject" or "person", as if the picture were some kind of printing mistake, you might get more credulous responses.

These are very minor crits. I'm looking forward to seeing some nice trophies Real Soon.

@bohigal, woops - hadn't noticed them, and yeh I agree with the bunny thing, all changed in Version 2, uploading now and downloadable from the original post once my internet connection decides to speed up.

When I printed it out here at work many of the questions were very faint. When a lads scans in his completed form there is no way you will be able to read the questions

An excellent document.
Now nit-picks :
Page 1 offense should be offence
decleration should be declaration
It mentions not returning the appendices, yet this is only the one (at the moment)
Page 2 sign found on Appendix 'A' change the 'on' to 'in'
'in the space provided below' but the space is on page 1?
'you may also attach the photo over this text' I would suggest getting rid of that line
Page 3 - several 'offenses' ought to be 'offences'
Page 5 - an furry animal - should be 'a' furry animal?
A suggestion for page 6 - change 'signatory' in bold black for a faint orange 'print your name here' in a large box so we will be able to read it.

I'll go get back in my box now

@freddie: agree with you on all points, except the page 1 photo was meant to be a passport style photo, just head and shoulders, whereas page 2 was meant to be a bigger one of the mugu holding up the sign. Are 2 photo's overkill? Maybe I should change page 1 photo to another fingerprint? I'll make changes soon.

These forms look great. You should consider perhaps setting one up for the US if you get the time!

I don't think two photos are overkill. The first one is going to be from their passport or something they use all the time, so it'll be no hassle for them. I WOULD recommend, however, a fingerprint page. Check out this one I used for the :

2 photos would be fine, instructions explaining the second photo could go into that space at the bottom of the relevant page along with a 'Stick photo here' box (use pins or staple, do not glue)

An American, Canadian or Australian would simply need revised Govt. logos and a spell check for the appropriate language, and an adaptation of the Law mentioned to suit the 'new' country.

Great form.

May I tactfully suggest the addition of 3 extra superflous pages to give it that extra bureacratic feel.

http://www.hm-treasury.gov.uk/media/8/0/combat_terrorism.pdf is a great example of legitamate UK paperwork with the copyright notice on page 2, blank page, and pages 1+4 being essentially the same thing.

Great work, look forward to using it on a pet soon.
