How come that little icon in the address bar changes

Used to be a NO 419 sign and today we have a book icon? And I have seen some others too

Having symbols change is often a sign of a video cache overload.

Occasionally the wrong icon gets assigned to a url. I have a few hundred bookmarks, roughly 2 % of them are now showing a wrong icon. Very likely this is browser-related on your end, the "favicon.ico" didn't change. Deleting the cache and (re-)bookmarking the page usually helps.

And how does that icon get there in the first place?
I'd like to put a unique icon in the address bar when people visit my website. How is this done?
Is it a .gif or .bmp that's referenced in the html??

It's a 16x16 pixel icon named favicon.ico that you put in the root directory of your website, no need to link it although you can.
