fashmo, there's also http://winformr.biz.ly/Congratulations.htm, which is the page people get redirected to after filling in the form hosted on the page you sent the abuse for.
Charles Soludo didn't just complete the form giving loads of fake info and putting "Yes" in the box marked "Sex", so he didn't get redirected to the congratulations page Wink

I have never filled any of the forms that i have received and believe me i have had my fair share of the forms. I may try that in the future and the link that you provided is already showing 404ed.

Mind you i have recieved 24 copies of the terms from sisal quite a professional document.
