I recieved this and it includes a google documents form to fill out all my paypal and bank details. Is it safe to fill out the form with fake answers? Or not worth the hassle? Second thing is who/where do I report it to get it shut down?

It's probably phishing (which we don't really deal with here), and the site may well have malicious software as a bonus.
You can report that to Paypal following the instructions here. They'll make short work of it. |

I would not download anything from someone or some place you are not sure of. You could get a nasty infection. |
