Hello,

As i felt to be more comfortable with the idea my business partner is trustworthy i have requested the dept. of 3xp0rt and Auth3nt1c1ty for a form to measure the trustworthiness.

Please review this and let me know if the dept. has forgotten anything.

Consider adding:

"Do any of the following cause you sexual excitement:

Ears (y/n)
Feet (y/n)

Small piece of advise: try to ask more open questions instead of circling a yes/no. Open questions take much more time to answer. Have him answer and explain every single one Laughing

I just fantasized my questionairie with logo's.

I use Adobe photoshop for the logo's and adobe pro for the pdf.

A very nice form. I'll have to give a lad this one, even if my character has to get themselves stredded in Amsterdam to do it.

Just a couple of points:

The Country of Birth and Nationality headings are strangely not italicized.
Q4 needs a capital letter.
Consider a bigger gap between Q42 and Q43
